OMMP Standard Doctor Visit

Looking to obtain your OMMP card for the first time or renewing your current card and have medical documentation of your qualifying condition? This is the appointment for you. Sign one of our release of information forms for us to obtain your medical records, or submit your current records, that you have already obtained yourself, directly to us. The soonest our office has your medical documentation, the sooner we can add you to our schedule. Keep in mind that it can take 30 days to obtain your records. All doctor appointments are secured with a $50 deposit at the time of scheduling, while the remaining balance is due on the day of your scheduled appointment. Contact us for more information.
